Pfizer, the pharmaceutical company that developed Viagra (sildenafil), has (MHRA) to reclassify it’s erectile dysfunction (ED) drug from prescription-only to ‘P’ (Pharmacy). This would allow pharmacies to sell sildenafilover the counterwithout a prescription.

The application is forsildenafil 50mg onlyin a maximum pack size of 8 tablets. Sildenafil tablets are available on prescription in strengths of 25mg, 50mg, and 100mg in pack sizes of four and eight tablets.

The move follows the in 2013. After the patent expired other pharmaceutical companies, apart from Pfizer, have been permitted to supply their own sildenafil tablets, sometimes known as generic Viagra.

The application has support from the pharmacy industry: “Sildenafil is one of the most counterfeited medicines,” said Royal Pharmaceutical Society president Martin Astbury, with many men “resorting to the internet to buy medication”.

Pfizer is committed to patient safety, and to ensuring that non-prescription sildenafil is supplied in a way that supports patient safety and improves patient health. Pharmacists are well placed to provide men with helpful and appropriate guidance to manage their erectile dysfunction, and should refer men to a doctor if they believe further investigation is required.

Sildenafil is already available over the counter in New Zealand since 2014 and Poland since 2016. The MHRA are considering the UK application and will make its decision within a few months (June-August 2017).

What does this mean for patients?

Many men suffering from ED will welcome easier access and availability of advice and treatment. If the MHRA approve the application, men will be able to buy Pfizer sildenafil in pharmacies without a prescription, including pharmacies in supermarkets (eg Tesco, ASDA). The Pharmacist would need to ask, likely in a private room, and may take a blood pressure reading.

According to the application,only men currently prescribed sildenafil 50mg(from a doctor or pharmacist with specialist training) will bepermitted to buy sildenafil 50mg over the counter.

The application to reclassify sildenafil applies to 50mg tablets in packs of 2 tablets (in the familiar blue diamond shape), according to the packaging submitted in the application. It is not yet clear how many packs or tablets men will be able to buy at one time, but the application stipulates a maximum pack size of 8 tablets.

If the application to reclassify is successful, prices are unlikely to come down straight away, although Pfizer is yet to announce a recommended price for over the counter sildenafil. Sildenafil produced by Pfizer for supply on prescription is priced significantly higher than that of other manufacturers of sildenafil (generic Viagra). If Pfizer is successful, other generic drug manufacturers are likely to also apply for their sildenafil products to be reclassified as P medicine status, at which point patients might see a reduction in price for over the counter sildenafil.

Many men will require a lower or higher dosage than 50mg. It is recommended to use only the minimum effective dose to mitigate potential side effects, and for men with mild erectile dysfunction symptoms this will be 25mg. Conversely, many men require 100mg to be effective, and in our experience the overwhelming majority request the higher dose, although because are scored they can be broken into half doses.

The Food and Drug Administration says more than 80 million men age 65 and older suffer from erectile dysfunction, a serious condition that affects nearly 3.5 million Americans.

But with the rise of online drug prices, men who buy prescription drugs such as Viagra and Cialis are getting a lower price.

"In the United States, we are seeing a significant rise in the prescription drug price and the pharmacy benefit manager (PBM) as a result of the rise of online pharmacy prices," said Dr. Michael J. Terrone, an American urologist who treats men with erectile dysfunction.

Terrone, who directs the Center for Sexual Medicine at the University of Texas Health Science Center at Fort Meer, Texas, said the rise in prescription drugs is a result of the internet's increased accessibility and the rising prices of drugs.

He said the increase in the price of drugs is the result of the rise in the online pharmacy industry.

"It is not the price of something you buy at the pharmacy, it's the price of the product itself," said Terrone, who is also a partner at a law firm in Dallas.

"That's why we are seeing an increase in the price of prescription drugs, that's why people are getting cheaper medications and not getting their prescriptions filled at the pharmacy," he added.

Terrone said he did not know how much of the increase in the price of drugs has been due to the increasing online pharmacy prices.

"It is an open-ended question. And in the online pharmacy, there is only a small percentage of customers, and you just have to look at what they are buying," he said.

The Food and Drug Administration said it has approved online pharmacies for sale by mail to physicians, pharmacists, dentists, and general practitioners.

A separate FDA report said the increase in the online pharmacy market is "a result of the increasing popularity and sale of prescription medications and the rise of online pharmacy prices," which are used to purchase prescription drugs and other prescription products.

Among the drugs listed in the FDA's online pharmacy database are generic Viagra, Cialis, Levitra, and Stendra, which are FDA approved, and are available at pharmacies across the country.

According to the FDA, generic drugs for erectile dysfunction are sold under the brand names Zydus Inc., Zydus Pharmaceuticals LLC, and Zydus International Inc. in 25 of the United States states.

For those customers, the price of prescription drugs will vary, depending on which pharmacy they choose to visit.

The price of a bottle of Cialis at a generic drug store will be $25. That's not the price of a prescription drug that's available at a pharmacy, Terrone said.

The price of Viagra, a Pfizer-sponsored drug that treats erectile dysfunction, will be $45. That's the lowest-cost version of the drug, and the lowest-priced version, the FDA said. The FDA also said the lowest-cost version of Viagra will be sold at a pharmacy.

The prices of generic Viagra and Levitra will also differ. Generic Viagra is sold at a lower price, and Levitra is sold at a higher price, according to the FDA.

The FDA also said the lowest-cost version of Cialis will be sold at a pharmacy.
